Care coordination is paramount for us, and we ensure our patients get the best care and all clinical teams are on the same page no matter the case! Job Requirements: Working days: Monday through Sunday Days vary off employee’s confirmed availability. Hours per week: Varies on the patient load an employee chooses to pick up. Can be adjusted as needed with proper and timely notification.
